If you live within our practice boundary you are welcome to register with us and our reception staff will be happy to guide you through the procedure, although there may be a surgery closer to you which you may want to consider. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill, along with one form of photo ID such as a passport or driving license. 

We accept new registrations during the following times each day: 10:00 - 12:00, and 13:00 - 16:00. If you arrive outside of these times you may be asked to return.

Following changes to the NHS Registration form we are no longer able to accept registrations via the website.

If you wish, please call into the surgery in person, and we will be happy to help you.

Medical treatment is available from the date of registration (which is when we register you, not when you hand the forms in. Registration can take up to one week) so please ensure you have sufficient medication before leaving your previous surgery.

You can drop into the surgery to collect the registration forms, or download the forms below to complete and print out. Every adult must present to the surgery with their forms, photo ID and proof of address. Your NHS number can be found on your NHS App, letters from an NHS service or your current GP surgery can provide this.

Please be advised that you must live within our boundary to register with us. We have quite a large boundary, which includes some NE2 postcodes, as well as NE4 and some NE5. We can register patients within the boundary running from the A1 Western bypass to Scotswood Road, all the way along to the new Redheugh Bridge, up Blenheim street on the A189 to Strawberry Place, Leazes Park and Queen Victoria Road, looping around on to the A167 of Jedburgh Road and going West to Moor Lane, North to Kenton Lane on the A191 and a small section of Ponteland Road on the A167 joining the A1 Western Bypass, just north of Etal Lane. If you are not sure whether your postcode is within our catchment area, please do not hesitate to contact us to ask.

Patient Charter

Our Expectations:

  • Please tell the practice of any change of address or telephone number
  • Children must be supervised at all times.
  • The building and car park has a strict no smoking policy.
  • Mobile phones should be switched off whilst on the premises.
  • Please display courtesy to our staff at all times – remember, they are working under our doctors' orders.
  • Respond in a positive way to questions asked by our reception staff.
  • Attending appointments on time or giving the practice adequate notice that you wish to cancel. Someone else could use your appointment!
  • Please engage with the practice when you are invited for your annual check; this check often acts as a medication review to ensure it is safe for you to continue on repeat medication.
  • An appointment is for one person only - where another member of the family needs to be seen or discussed another appointment should be made, and the Medical Record be made available.
  • You should make every effort when consulting with the surgery to make the best use of nursing and medical time - home visits should be medically justifiable and not requested for social convenience.
  • When you are asked to give 48 hours’ notice for repeat prescriptions, please give us this time as it is to allow for accurate prescribing.
  • Our surgery also exercises a Zero tolerance - this means:

Our staff come to work to care for others and it is important for all members of the public and our staff to be treated with courtesy and respect.

In line with the rest of the NHS, and to ensure this is fully observed, we have a Dignity at Work and Zero Tolerance Policy. This means that rude, aggressive or violent behaviour towards our staff or any member of the public within our practice premises will not be tolerated under any circumstance.

If this behaviour does occur, then this could lead to the police being notified and/or patients being removed from our list.
